My GM Care: Patient access to the GM Care Record
The My GM Care app gives access to the information held on your GM Care Record, including:
Medications – medications, allergies and vaccination history
My health – intolerances, family history, lifestyle
All about me – share what matters to you, e.g., care preferences, home access or pet information
Self-recorded measurements – blood pressure, weight, and mood measurements to keep you on track and share with health professionals
Care plans – digital care plans for dementia wellbeing, heart failure, frailty and end of life – view and update where possible
Remember to ask – store reminders about what to ask during an appointment
My Goals – create and monitor your health goals
